Government Moves Will Help Medical Homes and Accountable Care Organizations Succeed
By David K. Nace, MD

Robust primary care is essential to a high-quality, cost-efficient healthcare system. But in recent years primary care has been declining in the United States because of how we pay providers. Our fee-for-service, volume-based environment has encouraged too many providers to become specialists, while fewer younger providers are going into primary care. Urban Health Plan Becomes Patient Centered Medical Home

A recipient of the 2009 HIMSS Davies Community Health Award, Urban Health Plan (UHP) is a network of federally qualified community health centers based in the South Bronx and Queens. Its mission is to improve the health status of underserved communities. In 2008, UHP served 31,000 patients through 171,000 patient visits. It has served the Hunts Point, Mott Haven and other surrounding communities of the Bronx since 1974. As a federally qualified health center, UHP plays a vital role in providing health services to medically underserved populations who often have difficulty accessing quality healthcare. Now, UHP has earned recognition as a patient-centered medical home by the National Committee for Quality Assurance. In the video below, UHP's Daniel Figueras, chief technology officer, and Alison Connelly-Flores, clinical systems administrator, provide the components necessary to become a patient-centered medical home(PCMH).
