Advance Directives

Record whether a patient 65 years old or older has an advance directive. 

More than 50 percent of all unique patients 65 years old or older admitted to the eligible hospital's or CAH's inpatient department (POS 21) during the EHR reporting period have an indication of an advance directive status recorded as structured data.

Exclusion: An eligible hospital or CAH that admits no patients age 65 years old or older during the EHR reporting period. 

 

Measure Numerator

The number of patients in the denominator who have an indication of an advance directive status entered using structured data.

THRESHOLD:
The resulting percentage must be more than 50 percent in order for an eligible hospital or CAH to meet this measure. 

 

Measure Denominator

The number of unique patients age 65 or older admitted to an eligible hospital's or CAH's inpatient department (POS 21) during the EHR reporting period.
