Because of its potential for impacting quality of care and patient safety, patient identity management is a long standing challenge in the health care organization, provider office, or anywhere patient databases are collected. The increasing penetration of technology into the healthcare environment only exacerbates the problem. Recognizing the importance of this topic, HIMSS undertook efforts to address these issues. One outcome of this work was a White Paper on Patient Identity Integrity whose purpose is to identify the complex issues relating to the accuracy and completeness of electronic health data attached to or associated with an individual patient and the linking of all such data within and across systems. The paper discusses the critical business processes that must be in place to support and maintain the integrity of the data for quality of care, patient safety and cost management.

In researching for the White Paper, the HIMSS Work Group on PII discovered that there was no central source for information on the topic and gathering resources was difficult if they existed at all. HIMSS decided to develop a Toolkit on this topic as a central repository for available information to enhance the industry's understanding of patient identity integrity and the many issues involved in reliably – and safely – matching patient identity across systems. The Toolkit is divided into seven sections including: security safeguards, model interface protocols, key performance indicators, model data practices, model monitoring reports, executive level training, and literature reviews. These reflected the critical areas of recommendations made in the White Paper.
