Community Health Access Network (CHAN) - Davies Community Health Award

Community Health Access Network (CHAN)The Community Health Access Network (CHAN) is the only Health Center Controlled Network (HCCN) in New Hampshire.  Established in 1995, CHAN has developed and supports an integrated clinical and administrative system infrastructure for its ten Federally Qualified Health Center (FQHC) members which includes 3 Healthcare for the Homeless Programs.

Central to CHAN's focus has been the automation of the primary care health record.  CHAN's systems include a fully integrated Electronic Health Record (EHR), GE Centricity, which supports over 130 providers and partner hospitals.  The EHR also has links to member reference labs and four hospitals.

The EHR is linked to the GE Centricity Practice Management (PM) system and shares a common reporting tool.  The providers can also securely access the system remotely to support offsite care.

With EHR implementation, the network also planned for a decrease in CHC patient visit throughput time and looked to reduce the cost of chart audit while continuing clinical performance improvement. The hope was also that the EHR would be a strong recruitment asset for recently trained medical residents and nurse practitioners who expect the capabilities of EHR to aid them in managing their patients due to its inherent data organization, external linkages to and timeliness of information.

CHAN also anticipated faster turnaround of clinical information and feedback, e.g., lab results, aiding clinical staff in patient follow up and also reducing patient stress.
