Petaluma Health Center | Davies Community Health Award

Petaluma Health Center (PHC), the winner of the 2017 HIMSS Davies Community Health Award was founded in 1996 and became a Federally Qualified Health Center in 2000. PHC works to ensure access to high quality, prevention-focused health care to 35,000 patients each year in Sonoma County, California.
Dedicated to maximizing the value of health IT systems, PHC used their Electronic Health Record (EHR) and analytics platform to drive decisions, support care management of population health, and improve clinical outcomes.
PHC has received this award from HIMSS based on two use cases, which demonstrates their efforts to use EHR technology to improve patient health.
