Join the HIMSS National Capitol Area Chapter on October 21 at 5:30 pm ET for their webinar, “Complex Case Management Hub: Optimizing Value Based Patient Centered Care.” The Complex Case Management Hub (CCMH) is a single point liaison service and accountable care team designed to streamline and optimize outpatient care coordination for veteran patients with complex health conditions.
The CCMH reduces administrative burden on ordering providers, while maximizing efficiencies in specialty care to expedite the confirmation of diagnoses and the development of treatment plans by ensuring necessary prerequisite diagnostic testing, labs and imaging are completed and communicated to the Veteran's primary and specialty care teams through a complete comprehensive referral package.
