Creating a Community of Care:  Lessons from the Trenches

Over the last several years, the Milwaukee Health Care Partnership has focused on creating a collaborative, community-wide approach to improving health care for the low-income uninsured and Medicaid population of Milwaukee County, Wisconsin.  By leveraging the Wisconsin Statewide Health Information Network (WISHIN) and a shared on-line scheduling tool, and focusing on common goals, the Partnership has helped providers in its community improve patient care and improve quality of care. What lessons from their experience can be applied to insured populations to strengthen the success of alternative payment models?
