Accelerating Value-Based Care Transformation With a Unified Data Model

Tuesday, April 18 at 10:30 AM - 11:30 AM CT
South Building, Level 5 | S501

One of the key barriers to improve healthcare is useful information. We have oceans of data that are not connected and organized in ways that are actionable. What’s needed is access to centralized, shareable data that is interoperable with multiple applications. This session will highlight how Banner Health streamlined population health applications with intelligence layers to improve value-based care operations and reduce technology applications. Using an integrated, comprehensive, interoperable health cloud, Banner closed care gaps and implemented actionable, data-driven insights and implemented  population health dashboards aimed at improving value-based care.

Learning Objectives

  • Examine the challenges of large, multi-system facilities while aggregating and integrating data from multiple sources to implement a scalable infrastructure that meets the value-based care delivery operational needs
  • Demonstrate the role of a value-based care enterprise data fabric as a foundation to digitize operations, and improve patient outcomes, care workflows and application cost savings
  • Elaborate on the role of using analytics and predictive modeling in value-based healthcare to build a user-friendly, intelligent system that enables a connected framework to automate workflows throughout the care continuum
  • Illustrate the importance of using patient data to access actionable, data-driven insights aimed at assisting care teams to close care gaps and deliver more coordinated, convenient and customized value-based care
  • Highlight the outcomes achieved by unifying all data related to clinical, financial, utilization, risk and quality to create a 360-degree view of patients
