Social determinants of health (SDOH) are the conditions in which people are born, grow, live, work and age – conditions that can have a direct impact on our health. As strong advocates for improving SDOH nationwide, Highmark Inc. and the Blue Cross Blue Shield Association (BCBSA) are sponsors of the HL7 Gravity Project, a multi-stakeholder initiative working to standardize medical data used to identify SDOH. The project aims to develop national standards for SDOH data reported in electronic health records, population health systems and online referral tools for consumers, and is doing this by identifying data and critical code sets across four clinical and human service areas – screening, diagnosis, planning and interventions. This session will highlight BCBSA and Highmark’s commitment and progress on tackling the challenges of SDOH and using FHIR-based API technology, and how employers and the Covid-19 pandemic have accelerated the project forward.
