Social Care Interoperability for Older Americans: Innovation With Open Standards

This panel session will discuss these challenges and the associated standards being defined in various standards development organizations to improve information interoperability between healthcare and social care. We will also discuss various governmental and NGO challenges and pilots designed to encourage progress on implementation and demonstration of these standards. Finally we will discuss the lessons learned across a set of these challenges and pilots with special emphasis on the results for clients of the AgeSpan Area Agency on Aging during an ongoing social care and healthcare interoperability innovation pilot.

- Discuss the multi-tiered challenges of information sharing between social care, healthcare, and government agencies and the impact of those interoperability challenges on the delivery of services to address health related social needs
- Summarize how these interoperability challenges are being addressed by standards, such as the Gravity Project Social Drivers of Health (SDOH) Codes, FHIR SDOH Clinical Care Closed-Loop Referral Implementers Guide, and the HL7 Patient Administration National Directory Workgroup
- Assess how aligning with these standards, and the 21st Century Cures Act, can help healthcare and social care providers improve patients’ and clients’ access to services
- Evaluate and participate in innovative partnerships between clinical care, social care and technology companies federal government and non-governmental organization challenges and pilots which address these gaps in interoperability and drive better outcomes for patients of healthcare and clients of social care providers
