Hardeep Singh, MD MPH is a Professor of Medicine at the Center for Innovations in Quality, Effectiveness and Safety based at the Michael E. DeBakey VA Medical Center and Baylor College of Medicine, Houston. He leads a portfolio of multidisciplinary quality and safety research focused on measurement and reduction of diagnostic errors in health care, improving the use of health information technology and health system transformation. His research has informed national and international patient safety initiatives and policy reports, including those by the CDC, OECD and the WHO. He is an elected Fellow of the International Academy of Health Sciences Informatics for significant and sustained contributions to biomedical informatics and co-developed the "ONC SAFER Guides" which provide national recommendations for safe electronic health record use.  He represents the VA on the National Academy of Medicine’s Climate Collaborative and has proposed system-level approaches and strategies related to decarbonization of health care delivery. He recently co-chaired AHRQ and Institute for Healthcare Improvement’s Technical Expert Panel that developed “Reducing Healthcare Carbon Emissions: A Primer on Measures and Actions for Healthcare Organizations to Mitigate Climate Change”.  He has received prestigious awards for his pioneering work in quality, safety and health system transformation.
