SC House Calls
Scott Middleton holds a Master of Divinity from Emory University, and a Master of Health Administration from the University of South Carolina. In 1999 Scott founded Agape Senior which would grow to thirteen Assisted Living and Skilled Nursing facilities and the largest hospice in the Southeast by 2015. In 2015 he sold the Assisted Livings, Skilled Nursing Facilities, and hospice to invest his resources in a home care model of physician services, SC House Calls. SC House Calls model is a proactive, holistic approach to medicine relying heavily on technology to increase provider-to-patient touches in efforts to reduce cost and promote quality outcomes. In 2022, SC House Calls employed 300 providers serving 40,000 patients producing a savings of approximately $100 million for its traditional Medicare, Medicare replacement, and Medicare advantage programs. SC House Calls is currently a member of the Pearl Health ACO.
