Building Community-Based Solutions to Connect the Spectrum of Care
This session will bring insights from state leaders on the anticipated opportunities for expanded connectivity/interoperability across the spectrum of care – linking clinical, behavioral, public/population health and other non-health systems. The presenters will share state recommendations for creating nimble and adaptive systems that improve responsiveness and quality of care when addressing health crises such as environmental threats (lead), opioid abuse, and natural disasters (e.g. Hurricane Katrina, Joplin Missouri Tornados, etc).
