CHICAGO (October 16, 2017) – In expediting the benefits of their integrated electronic health record (EHR) systems, Unity Health Care vastly improved patient outcomes across their diverse patient population.
Many of Unity Health Care’s locations offer services to homeless women and men across the District to support population health for all—regardless of a person’s financial status, for instance.
The HIMSS Davies Award recognizes outstanding achievement of organizations that have utilized health information technology to substantially improve patient outcomes and value.
Such was the case with a 62-year-old homeless patient, who we’ll call Wilbur. Wilbur arrived at a Unity Health Care facility with both hypertension and degenerative disc disease. Wilbur was also unemployed and indigent, factors perhaps directly linked with his poor health conditions.
During consultation with Unity Health Care staff, Wilbur received a full health screening which detected both physical and mental health issues in dire need of treatment. Staff discovered Wilbur suffered from feelings of anxiousness, frustration, and limited activities due to pain and other health-related issues. He began working with a mental health clinician who met monthly with him. This provider coached him in pain management and encouraged him to participate in more activities that would benefit his overall health, both mentally and physically.

HIMSS Davies Award of Excellence
