On December 12, 2017 The Office of the National Coordinator for Health IT (ONC) announced the first Spotlight project for the Interoperability Proving Ground (IPG). The IPG is an open platform for sharing interoperability projects taking place within the United States and across the world.
The first Spotlight project is entitled Pharm2Pharm and highlights a collaborative care model that focuses on improving care coordination between hospital pharmacists and community pharmacists as patients move from the hospital to their home. A reduction in medication-related hospitalization rates in patients over 65 years or older was identified as a key finding in the project.
