Phoenix Community Health Center Selected as 2016 HIMSS Davies Award Recipient

Mountain Park Health Center, a Federally Qualified Community Health Center serving over 80,000 patients in the Phoenix, AZ area, has been named a 2016 HIMSS Community Health Davies Award recipient. Since 1994, the HIMSS Nicholas E. Davies Award of Excellence has recognized outstanding achievement of organizations that have utilized Health Information Technology (IT) to substantially improve patient outcomes while achieving return on investment. The Davies Awards program promotes Electronic Health Record-enabled improvement in patient outcomes through sharing of case studies and lessons learned across a wide range of efforts, including implementation strategies, workflow design, best practice development and adherence, and patient engagement that have improved ambulatory care outcomes for patients.

For more than 30 years, Mountain Park Health Center has worked with the communities it serves to sustain and improve health by providing quality, affordable primary care.  With five full service Joint Commission-certified Patient-Centered Medical Home clinics in the communities they are most needed, they provide a one-stop medical home for entire families. Services include pediatrics, women’s health, family medicine, dental, pharmacy, WIC, cancer screenings and health education classes.  They also have 3 pediatric clinics in partnership with a local YMCA and two local schools.  Mountain Park prides itself on taking care of people from all walks of life, and ensure patients are never turned away due to an inability to pay for care.

Mountain Park submitted three case studies highlighting how they utilized health information technology to enable improved cancer screening and early detection and reduce childhood obesity while not losing any provider productivity

“As a federally qualified community health center, Mountain Park has a diverse population with significant social determinants that contribute to the development of chronic disease.” said Jonathan French, Senior Director of Quality and Patient Safety at HIMSS. “Mountain Park has leveraged Health IT to standardize care and improve care plan adherence. These improvements have driven improved population health outcomes. The Davies Committee and HIMSS congratulate the team members of Mountain Park on winning the Davies Award.”

“At Mountain Park we are committed to helping our patients achieve the best possible outcomes through any means necessary,” said Mountain Park Community Health Center CEO, Dr. John Swagert.  “We are proud to be recognized with the HIMSS Davies Award of Excellence for our using Health IT for the benefit of our patients. Our efforts have helped our patients reduce childhood obesity, meet their health goals, and continue their journey towards good health.”

Mountain Park will be recognized at the 2017 Annual HIMSS Conference & Exhibition, February 19-February 23 in Orlando, FL.
