Cleveland Clinic Selected as 2017 HIMSS Enterprise Davies Award Recipient

CHICAGO (December 5, 2017) – Cleveland Clinic, a nonprofit multispecialty academic medical center, has been leveraging health information and technology systems for more than two decades; consistently keeping their finger on the pulse of all the latest digital innovations enhancing the value of patient care.

Real-time measurement and clinical decision support are the common themes for three Davies Award-winning use cases at Cleveland Clinic, the #2 ranked hospital in the United States.

Heart Failure Checklist

After identifying that preventable readmissions occur early, Cleveland Clinic developed an electronic Heart Failure Checklist in the electronic medical record (EMR) for patients diagnosed with heart failure. With this real-time checklist, Cleveland Clinic

• implemented an electronic “hard stop,” if all check list items have not been completed.

• reduced 30-day readmissions to the lowest levels ever seen for heart failure patients at Cleveland Clinic.

• sustained low readmissions over 3-year period.

With over 160,000 attributed adult patients suffering from hypertension, Cleveland Clinic recognized this as a critical issue, with high blood pressure ranking as the primary risk factor for premature death.

With evidence-based decision-making and standardizing best practice, Cleveland Clinic improved hypertension control and prevented

• 76 potential strokes.

• 51 heart attacks.

• 41 early deaths.

VitalScout SM

To better identify and manage patients exhibiting early signs of clinical deterioration, Cleveland Clinic developed VitalScout SM, an enhanced early warning system that scores vital signs based on risk for deterioration and provides color-coded alerts for at-risk scores. Innovative technology was developed to provide caregivers with tools to interact with this information, improve situational awareness, promote critical thinking, guide timely intervention, and increase accountability. With VitalScout SM Cleveland Clinic has

• decreased the time patients on non-ICU units spend with “at risk” vital signs.

• decreased the time to reassessment of vital signs following an alert.

• improved interdisciplinary communication and collaboration.

Leveraging real-time analytics from their electronic health record (EHR) at the point of care, Cleveland Clinic drove efforts to increase consistency in the care delivery model with these real-time, electronic tools.
