Social Determinants of Health and Virtual Observation Case Studies

Davies Award of Excellence
Provider addressing social determinants of health with virtual observation

Atrium Health was named a 2019 HIMSS Davies Enterprise Award recipient for leveraging the value of health information and technology to improve outcomes. The three case studies below cover how the organization implemented virtual observation solutions to reduce patient falls, improved opioid prescribing practices and reduced readmissions by addressing patients’ social determinants of health.

Case Studies

1. Patient Virtual Observer and Falls Management

Atrium Health implemented a virtual patient observation solution as a pilot for a rehabilitation hospital in 2016. The goal was to reduce the patient fall rate, decrease the number of 1:1 sitter hours, decrease the rental of fall preventing specialty beds, and lower restraint utilization. To do so, they implemented a 3D motion-tracking camera and falls prevention solution to keep a watchful eye on patients.

2. Improving Opioid Prescribing Practices

Through a collaborative effort, Atrium Health created an opioid toolkit to address prescribing patterns in opioid prescriptions. This included an alert system which helped provide critical information at the point of care. The ultimate goal of this intervention was to support clinical decision-making in the prescribing process and keep patients safe.

3. Reducing Readmissions Through Data-Driven Care

Analytically-driven, personalized care delivers value. Many times, the answer to the readmission question of “why” lives in the data and determining the root cause is part of the success in reducing readmissions at Atrium Health. Discussing the data with the front-line staff, the patients, and the families plays a key role in achieving a reduction in readmissions year after year. Utilizing paired workgroups, analytics, integrated EHR information, and a commitment to personalized standardization of care has proven to be a beneficial model for readmission reduction at Atrium Health.
