The Future of Precision Medicine | March 8, 2017 | 12:00 p.m. - 1:00 p.m.
Precision Medicine (PM) is a medical paradigm which integrates all data available on an individual patient, enabling a multifactorial approach to the tailoring of treatment to the individual.  For this discussion regarding the future of PM, we will apply a definition that includes "aligning and integrating diverse, unstructured, data sets into a comprehensive knowledge network”(Hawgood et al.).  The knowledge network drives PM in ways that enable physicians and patients to identify and implement tailored treatments and diagnostics by joining disparate data to answer complex questions. Carequality & The Health Data Sharing Tipping Point: Providers shared their experiences implementing the nationwide Carequality interoperability framework in a panel discussion. By leveraging existing investments, healthcare leaders tripled the number of care sites enabled to share information across the entire healthcare continuum in just the first 90 days. They shared best practices in implementing the leading interoperability framework, and insights for the future of health data sharing among and between providers, payers, and patients.

C-CDA Documents Provide the Building Blocks for Meaningful Exchange
C-CDA Documents are a promising vehicle for interoperability, though misconceptions are common. Read Health Story Project member Nick Mahurin’s blog post on these documents, which can be leveraged for more meaningful data exchange. Many know and utilize the Continuity of Care Document (CCD), but Nick points to other document types that can better serve specific use cases for exchange. Focus on Long-Term Good Health with Free Immunization Capability Testing
Three (3) organizations working together - CNI Advantage, HIMSS and ICSA Labs, now invite health IT vendors to test their immunization-capable products in the Immunization Integration Program [IIP] supported by CDC. Participation is free of charge. Vendors who participate in this program will demonstrate that their product includes software capabilities that address provider and patient reminders and up-to-date demographics and immunization information.
