The Story of a Patient Story That’s Making a Difference for HIT Standards

Lenel James and Lisa Nelson shared the story of their experience to date with the HL7 C-CDA Care Plan Document standard and its trial use in a multivendor team that included multiple EHRs and a Payer Care Plan Management System. They described the patient story at the center of this standards development and testing activity. They also described how this story may be leveraged as additional pilots with the digital Care Plan exchange are being planned.
