Intermountain Health Kidney Services (IKS) is a collaborative effort between Intermountain Health and MDClone to improve care for individuals with chronic kidney disease (CKD). This initiative involves data-driven AI-enhanced analysis to identify and stratify patients, along with a dedicated healthcare team offering education and advanced treatments. The collaboration deploys algorithms to accurately identify CKD patients by stage, enabling outreach and care coordination. MDClone and Actium Health developed a patient engagement tool for prioritizing and coordinating outreach efforts. The IKS Nephrology care team implements stage-specific action plans, education, and kidney-protective therapies. Despite challenges, the clinic's efforts have led to proper risk stratification, education, and treatment implementation. Notable outcomes include a high percentage of patients receiving staged diagnoses, increased use of ACE/ARB therapy, a reduction in admissions, and prevention of progression to dialysis for certain CKD stages.
